An Assertive Community Treatment (ACT) team consists of a community-based group of medical, behavioral health and rehabilitation professionals who use a team approach to meet the needs of an individual with severe and persistent mental illness. The role of the ACTT Nurse is to work as a team member responsible for conducting assessments, addressing physical health needs, making appropriate referrals to community physicians, providing management and administration of medication in conjunction with the psychiatrist, providing a range of treatment, rehabilitation, and support services.
Role and Responsibilities:
Provide service coordination (case management) for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team
Provide individual supportive therapy and symptom management, ensuring immediate changes are made in the treatment plans as clients need as well as advocating on behalf of client rights and preferences
Provide on-call crisis intervention covering nighttime hours and serve as a backup to evening and weekend staff
Organize and manage the system of getting medication to clients and integrating medication administration tightly into clients individual treatment plan
In collaboration with the team psychiatrist, coordinate, schedule, and administer medical assessments of client physical health, making appropriate referrals to community physicians for further assessment and treatment and coordinate psychiatric treatment with medical treatment
Basic Requirements:
Ability to perform tasks on the computer including working with Microsoft 360, Excel, Word and other basic programs.
Valid drivers license, auto insurance coverage and reliable transportation
Pass criminal background check and other regulatory checks
Provide proof of your education
Education and Experience Required:
Registered Nurse License (RN), Licensed Practical Nurse (LPN) or Nurse Practitioner
Minimum of one (1) year experience working with adults with serious mental illness and a working knowledge of psychiatric medications, regardless of team size.
Licensure/Certification Requirements: RN or Nurse Practitioner
Resume must include degree earned (major & minor), graduation month & year, relevant employment dates including month and year, and populations served.
