A nonprofit services organization in New York City is looking to add a licensed Registered Nurse (RN) to join their growing team. In this role, the Registered Nurse (RN) will be responsible for providing evaluation, diagnosis, and therapeutic intervention as well as monitoring treatment plans and progress.

About the Opportunity:

• Shift: Days
• Schedule: Monday, Tuesday, Saturday & Sunday
• Hours: 7am to 7pm
• Setting: Intermediate Care Facility
• Patient Volume: 12

Responsibilities:

• Review incoming referrals and update medical chart + medical tracking
• Schedule medical appointments
• Complete, quarterlies, semi annuals and annual CFAs
• Review and update individualized plan of Nursing services (PONS) annually and as needed
• Review and update Pressure Prediction and Prevention annually and as needed
• Review and update medical face sheet annually and as needed
• Review and update individual specific care instructions annually and as needed
• Review and update adaptive equipment annually and as needed
• Obtain and share hospital updates with the team; make hospital visits, as needed
• Perform other duties, as needed

Qualifications:

• Graduate of an accredited School of Nursing
• NYS Registered Nurse (RN) license
• CPR certification
• Solid assessment, clinical, and documentation skills
